weekend Saturdays 8a-8p and Sundays 8a-8p Friday 8p-8a Saturday 8p-8a. Responsibilities Monitor and observe patients closely to identify changes in condition and report findings accurately using our Hippa compliant software,clock in and out properly for client invoices for private payment. Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer techniques. redirection when needed in dementia cases. Administer medications safely following home healthcare regulations and medical orders, ensuring proper documentation and adherence to HIPAA confidentiality standards. Perform vital signs assessments—such as blood pressure, pulse, temperature, and respiration—and assist with specialized care like catheter management and Alzheimer's or dementia care routines. Support patients with memory care needs and end-of-life care by providing compassionate emotional support and ensuring comfort measures are maintained. Maintain a clean, safe environment by assisting with light housekeeping tasks such as laundry and meal preparation while adhering to infection control protocols. Communicate effectively with clients, family members, and interdisciplinary team members to coordinate care plans and ensure client satisfaction. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred. Demonstrated knowledge of medical terminology, medication administration procedures, and medical documentation practices. Hands-on experience working with individuals with developmental disabilities or those requiring dementia or Alzheimer's care. Familiarity with EMR (Electronic Medical Records) systems and home health software for accurate recordkeeping. Proven ability to perform patient transfer methods safely using equipment like Hoyer lifts or other mobility aids. Experience working within home health care regulations, hospice or palliative care settings is a plus. Valid certifications such as CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) are required; additional certifications in first aid or hospice care are advantageous.
