Summary of Responsibilities:

The Hospice Aide is responsible for personal care services as determined by the Plan of Care, in accordance with a physician's written orders and under the direction and supervision of a registered nurse. Principle duties and accountabilities include, but are not limited to: 1. Delivers personal care services for each assigned patient per the Hospice Aide Assignment developed by the RN Case Manager. 2. Assures all services indicated in the Hospice Aide Assignment are provided, including personal care needs of patient, assistance with light duty household tasks, and/or assistance with exercise, ambulation or transfer needs. 3. Participates in IDT as needed for quality patient care. 4. Reports to the RN Case Manager any appearance and/or behavioral changes in the patient's condition and requests changes in Hospice Aide Assignment as needed by patient/family. 5. Completes all required documentation. 6. Complies with the state and federal regulatory requirements.

Qualifications:

1. Current Home Health Aide/Certified Nurses Assistant Certification. 2. Complies with acceptable professional standards and practice. 3. Demonstrates good communication. 4. Requires reliable transportation, current driver's license and current auto insurance.
