Job Description

Encompass Health is seeking a PT Team Leader to guide an interdisciplinary rehabilitation team in our inpatient setting. In this role, you will coordinate daily patient care, oversee staff assignments, and support exceptional outcomes for patients recovering from major illnesses and injuries. You will collaborate closely with physicians, therapists, and nurses to develop and implement individualized care plans, ensure regulatory and safety standards are met, and promote best clinical practices. You will mentor team members, support professional development, and contribute to a culture of integrity, respect, and continuous improvement while advancing your leadership skills in a clinically advanced, patient-centered environment. Responsibilities Lead and support a multidisciplinary rehab team to deliver patient-centered care Coordinate daily patient care activities and staff assignments Collaborate with physicians and clinicians to develop and update care plans Monitor clinical quality, safety, and regulatory compliance Coach, mentor, and develop team members to support professional growth Facilitate effective communication across disciplines and with families Assist with staff onboarding, training, and performance feedback Use EHR systems to review documentation and support care coordination Participate in quality improvement and patient experience initiatives Support scheduling and workload balance to meet patient needs Required Skills Team leadership Patient care coordination Rehabilitation planning Clinical documentation Interdisciplinary collaboration Care plan evaluation Staff scheduling Quality and safety compliance Electronic health record (EHR) use Performance coaching
