In this role, you will provide essential support to patients in a hospice setting, ensuring their comfort and dignity during end-of-life care. The ideal candidate will possess a strong background in caregiving and have experience working in various healthcare environments such as nursing homes, medical offices, or home care settings. Responsibilities Assist patients with daily living activities, including bathing, grooming, and dressing. Administer medications as prescribed and monitor patient responses. Provide companionship and emotional support to patients and their families. Document patient care activities accurately in medical records using systems like Epic. Collaborate with healthcare professionals to develop and implement personalized care plans. Maintain a clean and safe environment for patients by following infection control protocols. Support patients with mobility and physical therapy exercises as directed by nursing staff. Communicate effectively with team members regarding patient status and any changes in condition. Requirements Previous experience in caregiving or related fields such as nursing homes, clinics, or home community care is preferred. Familiarity with medication administration and basic medical procedures is essential. Strong interpersonal skills with the ability to provide compassionate care to patients and families. Experience working with children is a plus but not required. Ability to maintain accurate medical records and documentation practices. Must be reliable, punctual, and able to work flexible hours as needed. Certification as a Home Health Aide (HHA) or similar qualification is desirable but not mandatory.
