Responsibilities Deliver high-quality patient care by assisting with activities of daily living, including bathing, dressing, feeding, and mobility support using equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, temperature, pulse, and respiration rates, ensuring precise medical documentation within electronic health record (EHR) systems. Support clinical teams by preparing patients for procedures, assisting with medical documentation, and maintaining aseptic techniques during treatments. Administer medications and perform basic nursing tasks under supervision while adhering to HIPAA regulations to maintain patient confidentiality. Assist in patient transfers using appropriate methods and ensure safe manual handling practices to prevent injury. Observe patients closely for changes in condition or behavior, especially in specialized areas such as memory care or behavioral health units. Collaborate effectively with interdisciplinary teams across settings like long-term care facilities, hospitals, outpatient clinics, hospice, or home care environments to ensure comprehensive patient support. Experience Proven experience working as a CNA or in similar caregiving roles within hospital medicine, long-term care facilities, assisted living communities, or home & community care settings. Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation and record management. Experience providing specialized care including Alzheimer's/dementia care, memory care, hospice support or working with individuals with developmental disabilities. Knowledge of medical terminology and procedures related to acute and chronic conditions such as infectious disease management or ICU-level care. Demonstrated ability to perform vital signs measurement accurately and execute safe patient transfer methods using equipment like Hoyer lifts. Strong understanding of HIPAA regulations and maintaining patient confidentiality at all times. Additional experience in pediatrics or geriatrics is highly valued; certification in basic life support (BLS) or CPR is required.
