Care Manager:

Overview of Job Responsibilities:

Developing and implementing care plans with program participants and supporting their efforts to attain their goals Complying with all relevant regulations governing the program Connecting program participants to community resources and housing options Modeling positive and appropriate behaviors and maintaining professional boundaries To qualify you must have: A Bachelor's Degree in a human services field Two years of experience in a related human services field An ability to operate independently, think on your feet, and follow your training Excellent organizational and time management skills Strong interpersonal boundaries Our shared belief system in areas such as compassion, respect, positivity, and inclusion Successful completion of RISE background checks, a valid driver's license and a driving history acceptable to the RISE automobile policy.
