• Care Manager will coordinate services and support members involved in child welfare and related systems of care.

This role requires frequent in-person visits, telephonic outreach, assessments, team collaboration, and timely completion of required documentation. The position is unique due to state requirements for children in foster care and the quick timelines associated with placement changes. Typical Day in the Role Conduct 48-hour in-person visits following a placement change for the member. Participate in 7-day team meetings following a placement change for the member. Conduct telephonic outreach based on member acuity. Conduct in-person visits based on member acuity. Complete annual assessments. Conduct in-person visits that may take place after 5:00 PM. Participate in weekly team meetings. Complete the Health Risk Assessment and Plan of Service required for each member. Ensure required in-person visits and member outreaches are completed within established timelines. Maintain high-quality documentation and ensure all required activities are completed accurately and timely. Performance will be measured based on completion and timeliness of required assessments, plans, visits, and outreaches. Education/Certification Master's degree in Behavioral Health or Social Work, or a degree from an Accredited School of Nursing. 2-4 years of related experience required. Licensure Licensed Master's Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN based on state contract requirements. Behavioral Health experience is required. Minimum Qualifications At least 1 year of experience coordinating services across child welfare and related systems of care. Experience working with children in foster care and/or child welfare systems. Strong communication skills. Ability to produce high-quality documentation. Adaptability and flexibility in responding to changing member needs. Solution-focused approach to addressing challenges. Ability to manage required visits, outreaches, assessments, and documentation within strict timelines. Ability to travel extensively throughout the assigned service area. Disqualifiers Candidates without experience working in child welfare and related systems of care will not be considered. Additional Qualities to Look For Strong communication skills.
