Job Description

Job Summary We are seeking a dynamic and strategic Home Health Administrator/Supervising Nurse to lead and oversee the operations of our home health services. The Home Health Administrator/Supervising Nurse provides overall administrative and clinical leadership for the Home Health Agency, including daily operations, patient care services, personnel, quality improvement, regulatory compliance, financial and operational performance, and coordination with hospital and community healthcare services. This position is responsible for ensuring the agency provides safe, effective, patient-centered, high-quality home health services in compliance with applicable federal and state laws, Texas HCSSA requirements, CMS Medicare Conditions of Participation, Medicare/Medicaid requirements, payer requirements, HIPAA, OSHA, accreditation standards, and organizational policies. As part of a hospital-owned rural healthcare organization, the Administrator/Supervising Nurse works closely with hospital leadership, medical staff, clinical departments, patients, families, and community providers to support continuity of care, patient access, quality outcomes, regulatory compliance, and the organization's mission.
ESSENTIAL RESPONSIBILITIES
Regulatory Compliance & Survey Readiness Maintains agency compliance with applicable Texas HCSSA licensing requirements, CMS Conditions of Participation, Medicare/Medicaid requirements, HIPAA, OSHA, accreditation standards, payer requirements, and organizational policies. Maintains continuous survey readiness and ensures required policies, personnel records, clinical records, logs, reports, licenses, certifications, credentialing documentation, and other regulatory records are current and maintained. Coordinates and participates in HHSC, CMS, accreditation, complaint, validation, re-credentialing, payer, and other regulatory surveys, audits, and reviews. Serves as an administrative liaison with surveyors and regulatory representatives, as appropriate. Addresses identified deficiencies and ensures timely development, implementation, monitoring, and completion of Plans of Correction. Monitors regulatory changes and implements necessary operational, clinical, and policy changes. Patient Care & Clinical Operations Provides administrative and clinical leadership and oversight of patient care and clinical services. Serves as the agency's Supervising Nurse and provides clinical oversight consistent with applicable Texas HCSSA requirements and scope of practice. Ensures skilled nursing, therapy, and other authorized services are provided according to physician/practitioner orders, plans of care, patient needs, regulations, and agency policies. Ensures patients are accepted for services only when the agency has the staffing, resources, competency, and capacity necessary to safely and appropriately meet their needs. Oversees processes related to referrals, admissions, transfers, discharges, recertifications, missed visits, changes in condition, hospitalizations, and resumptions of care. Promotes coordination and continuity of care among patients, families, physicians/practitioners, nursing, therapy, aides, hospitals, pharmacies, DME providers, and other healthcare professionals. Oversees clinical documentation, clinical record review, record retention, confidentiality, release of information, and HIPAA/privacy requirements. Participates in direct patient care when necessary and within the individual's scope of practice. QAPI, Quality & Patient Safety Provides administrative and clinical oversight of the agency's Quality Assessment and Performance Improvement (QAPI) program. Reviews, trends, and analyzes agency performance and quality data to identify risks, measure outcomes, and support continuous improvement. Monitors quality indicators including hospitalizations, emergency department utilization, adverse events, infections, falls, wounds, complaints, grievances, incidents, medication concerns, documentation deficiencies, missed visits, and other applicable quality indicators. Facilitates corrective actions and performance-improvement initiatives and evaluates their effectiveness. Promotes patient safety, accountability, regulatory compliance, quality improvement, and prevention of adverse events. Personnel & Staffing Ensures the agency employs or contracts with qualified, licensed, competent, and appropriately trained personnel. Oversees staffing, scheduling, assignments, workloads, orientation, competency assessment, education, evaluations, professional development, and employee performance. Participates in recruitment, interviewing, selection, hiring, counseling, discipline, and termination in accordance with organizational policy. Ensures personnel files contain required licenses, certifications, competency documentation, education, evaluations, background checks, exclusion checks, and other required records. Promotes effective communication, accountability, professional development, and interdisciplinary teamwork. Policies, Patient Rights & Risk Management Develops, implements, reviews, and maintains agency policies, procedures, and operational practices consistent with regulatory and organizational requirements. Ensures patient rights are protected and oversees the receipt, investigation, resolution, documentation, and trending of complaints and grievances. Ensures incidents, adverse events, abuse, neglect, exploitation, and other reportable events are appropriately investigated, documented, reported, and followed up. Oversees emergency preparedness, infection prevention and control, employee and patient safety, and risk-management activities. Financial & Operational Management Participates in development, implementation, and monitoring of the agency operating budget. Monitors census, admissions, referrals, discharges, staffing utilization, productivity, payer mix, resource utilization, and other operational indicators. Collaborates with billing and financial personnel regarding reimbursement, authorizations, documentation, and operational concerns. Promotes appropriate use of personnel, supplies, equipment, and financial resources while maintaining quality and regulatory compliance. Ensures public information and marketing materials accurately represent services the agency is authorized and capable of providing. Leadership & Collaboration Keeps the CEO and executive leadership informed of significant clinical, operational, financial, staffing, compliance, patient-safety, and regulatory matters. Maintains effective communication with patients, families, employees, physicians/practitioners, hospital personnel, community providers, payers, and regulatory entities. Promotes effective coordination between Home Health and other hospital and healthcare services. Participates in management, quality, interdisciplinary, and organizational meetings as required. Designates and maintains a qualified Alternate Administrator to provide administrative coverage during periods of absence.
QUALIFICATIONS
Education Bachelor's degree in Nursing, Healthcare Administration, Business Administration, Public Health, or a related healthcare field preferred. Graduate of an accredited school of professional nursing preferred. Required Licensure & Administrator Qualifications Current, unrestricted Registered Nurse (RN) license in the State of Texas is required. Must meet all applicable Texas HCSSA requirements for both Administrator and Supervising Nurse qualifications. Must have at least one (1) year of management or supervisory experience in a health-related setting, consistent with applicable Texas HCSSA Administrator requirements. Must complete all required Texas HCSSA Administrator training and continuing education. Prior to designation as a first-time Administrator, must complete required educational training applicable to the administration of a licensed Home and Community Support Services Agency, including applicable state and federal laws and licensing standards. Maintains all licenses, certifications, training, and continuing education required for the position. Current driver's license and automobile liability insurance required when travel is necessary. Current CPR certification required when direct patient care responsibilities are performed or otherwise required by agency policy. SKILLS & PROFESSIONAL REQUIREMENTS
Demonstrates strong leadership, organizational, critical-thinking, problem-solving, and communication skills. Maintains working knowledge of home health operations, regulatory compliance, quality improvement, clinical documentation, reimbursement, personnel management, and healthcare operations. Must be able to manage multiple priorities, exercise independent judgment, maintain confidentiality, and respond appropriately to urgent clinical or operational situations.
PHYSICAL & MENTAL REQUIREMENTS
Duties require prolonged periods of sitting, computer use, and concentration, with frequent interaction and communication with patients, employees, providers, and other individuals. The position requires the ability to exercise sound judgment, prioritize and manage multiple responsibilities, respond appropriately to changing circumstances, and perform detailed work accurately. Duties may occasionally involve lifting or carrying up to approximately 25 pounds, bending, stooping, squatting, twisting, reaching, travel, and handling equipment. SCHEDULE
Monday - Friday 8:00 am to 5:00 pm with on-call hours as scheduled.
