NOC Licensed Vocational Nurse (LVN) / Charge Nurse Job Summary Yucaipa Hills Post Acute, formerly Calimesa Post-Acute, is an 82-bed skilled nursing facility seeking compassionate, dedicated, and motivated Licensed Vocational Nurses (LVNs) for our PM shift. We are looking for a dynamic and compassionate PM LVN Charge Nurse to lead our nursing team in delivering exceptional resident care. This vital role combines clinical expertise with leadership responsibilities to ensure high standards of safety, efficiency, and patient-centered service. As a Charge Nurse, you will coordinate daily nursing operations, oversee clinical procedures, and mentor staff while promoting a positive, collaborative, and compliant work environment. Your leadership will help support quality care for residents with complex medical needs, including Alzheimer's disease, memory impairments, dementia-related conditions, and chronic conditions. Stay proactive and on top of resident care responsibilities. Provide leadership that promotes patient safety, quality care, and team collaboration. Maintain a positive and respectful work environment. Responsibilities Lead and supervise nursing staff during assigned shifts. Ensure adherence to clinical protocols, facility policies, and safety standards. Coordinate resident assessments, vital signs monitoring, and physical examinations. Assist in developing and following individualized resident care plans. Administer medications and treatments according to physician orders and facility policy. Respond to urgent or emergency situations and assist with appropriate triage and care. Collaborate with the interdisciplinary team regarding admissions, discharges, and resident care needs. Communicate effectively with residents, families, staff, and management. Maintain accurate documentation using EMR/EHR systems. Promote infection control practices, resident safety, and quality care standards. Experience and Qualifications Active LVN license in the State of California required. Current BLS Certification required. Prior experience in a patient care setting is preferred; however, we are willing to train the right candidate. Experience working with individuals with developmental disabilities, Alzheimer's disease, dementia, memory impairments, or chronic conditions is preferred. Strong understanding of the nursing process, including assessment, planning, implementation, and evaluation. Hands-on experience with EMR/EHR systems preferred. Knowledge of anatomy, physiology, pharmacology, medication administration, pain management, infection control, respiratory support, catheterization procedures, and resident care practices preferred. Familiarity with skilled nursing, assisted living, home health, or community care environments is highly desirable.
