2996669121-2 Apply for this job now Job Description MidState Medical Center, part of Hartford HealthCare, seeks an RN Clinical Coordinator to support coordinated, patient-centered care on our acute care units. In this role, you'll blend direct bedside nursing with leadership, overseeing daily clinical operations, staffing, and workflow while serving as a resource to the team. You'll champion evidence-based practice, quality, and safety, support staff development, and collaborate closely with providers and disciplines to deliver compassionate, high-reliability care to our diverse central Connecticut community. Responsibilities Coordinate and oversee daily nursing care on assigned unit, ensuring safe, high-quality, patient-centered services Support clinical workflow, staffing, and patient assignments in collaboration with nurse manager and interdisciplinary team Provide direct RN care, including assessment, care planning, implementation, and evaluation for diverse patient populations Serve as a clinical resource and mentor for bedside nurses, supporting evidence-based practice and protocols Monitor quality, safety, and regulatory compliance indicators; assist with audits, chart reviews, and performance improvement Facilitate patient and family education, care conferences, and smooth transitions of care and discharge planning Collaborate with physicians, advanced practice providers, and ancillary departments to optimize patient outcomes Assist with onboarding, training, and competency validation for new and existing staff nurses Use the electronic health record and clinical technology accurately to document care and support decision-making Participate in unit-based councils and initiatives focused on continuous improvement and patient experience Required Skills Acute care nursing Patient assessment and clinical judgment Care coordination and discharge planning Electronic health record (EHR) documentation Clinical leadership and team coordination Evidence-based nursing practice Quality and patient safety monitoring Interdisciplinary collaboration Patient and family education Time management and prioritization
