We are looking for a Care Manager, RN for our Connecticut team! This is a field-based position that requires frequent travel to patient's homes in New Haven to West Hartford, CT. Overview The Care Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care. This role collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes. Responsibilities Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed. Develops, implements, manages patient's plan of care. making revisions and documenting changes as needed. Actively coordinates the care of the client through communication and written documentation to the physician, and other disciplines involved in the client's care and community resources. Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to appropriate resources. Reviews the caregiver notes daily and appropriately addresses changes in the consumer's condition or environment. Ensure continuous compliance with all regulatory standards as well as company policies and procedures. Performs other job-related duties as may be required or directed by the supervisor. Home care case management Care coordination experience Qualifications Valid and active Registered Nurse (RN) license in the State of Connecicut Ability to articulate cases through effective case summaries. Ability and flexibility to travel (usually by car) within assigned geographic area; valid driver's license and car insurance required. Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others.
