Transitional Care Unit Nurse Manager -
RN The Lutheran Home Skilled Nursing Community Salary:
$95,000 - $113,000 annually, depending on experience Help guide residents through successful rehabilitation and transitions of care in a leadership role that combines clinical expertise, collaboration, and resident advocacy. Lutheran Home is seeking an experienced Registered Nurse to oversee care coordination within our skilled nursing and rehabilitation setting. In this role, you will partner with interdisciplinary teams to support resident recovery, discharge planning, and quality outcomes while ensuring exceptional clinical care. Key Responsibilities Coordinate care for residents receiving short-term rehabilitation and skilled nursing services Assist with admission assessments, care planning, and discharge preparation Collaborate with physicians, therapists, nursing staff, social services, and families to support individualized care Identify and address changes in resident condition to promote positive outcomes and reduce avoidable hospital transfers Provide guidance and education to residents, families, and team members regarding treatment plans and recovery goals Monitor documentation, care delivery, and regulatory compliance standards Support operational and clinical initiatives within the rehabilitation program Participate in an on-call rotation to assist with resident care needs Qualifications Required Current Illinois Registered Nurse (RN) license Minimum of two years of nursing experience in skilled nursing, rehabilitation, post-acute care, or long-term care Experience coordinating resident care and collaborating across multiple disciplines Strong communication and organizational skills Preferred Nurse leadership, supervisory, or care management experience Knowledge of post-acute rehabilitation and transitional care practices Ability to effectively prioritize and manage competing responsibilities Benefits & Perks Medical, dental, and vision coverage 403(b) retirement savings program Daily Pay option Paid time off Tuition assistance Employee wellness and mental health resources Chaplain support services Professional growth opportunities through our career development programs Schedule Full-time Day shift Weekend on-call rotation required Join Our Team We welcome dedicated nursing professionals who are passionate about helping residents achieve their highest level of independence and quality of life.
