00 Carroll Park Healthcare is a skilled nursing facility on the rise, and we are seeking an exceptional candidate to join our dynamic nursing leadership team. We are currently hiring a full-time MDS Coordinator. The MDS Coordinator (Nurse Assessment Coordinator) is responsible for coordinating and completing the Minimum Data Set (MDS) process in accordance with federal and state regulations. This position plays a critical role in supporting Quality Assurance and Performance Improvement initiatives, driving positive clinical outcomes, and ensuring care planning accurately reflects each resident's individual needs. Primary Duties and Responsibilities:
Coordinate and complete MDS assessments in compliance with federal and state regulations Ensure timely and accurate submission of MDS data and supporting documentation Lead and participate in care plan development to reflect individualized resident needs Monitor resident status from admission through discharge to ensure appropriate assessment and reimbursement accuracy Collaborate with interdisciplinary teams to address clinical, psychosocial, and administrative needs Support QAPI initiatives by identifying trends, opportunities for improvement, and ensuring data integrity Maintain accurate and complete clinical records, including proper utilization of PointClickCare (PCC) Communicate effectively with residents, families, and staff to ensure concerns are addressed promptly Investigate and assist in resolving resident and family concerns in a professional and timely manner Promote a culture of dignity, respect, and individualized care for all residents
Requirements:
Current Maryland RN or LPN license (or compact/multi-state license) Current BLS/CPR certification Strong knowledge of MDS processes, nursing care practices, and regulatory requirements
Preferred Qualifications:
MDS Certification (AANAC)
