Primary Duties and Responsibilities:
Coordinate and complete MDS assessments in compliance with federal and state regulations Ensure timely and accurate submission of MDS data and supporting documentation Lead and participate in care plan development to reflect individualized resident needs Monitor resident status from admission through discharge to ensure appropriate assessment and reimbursement accuracy Collaborate with interdisciplinary teams to address clinical, psychosocial, and administrative needs Support QAPI initiatives by identifying trends, opportunities for improvement, and ensuring data integrity Maintain accurate and complete clinical records, including proper utilization of PointClickCare (PCC) Communicate effectively with residents, families, and staff to ensure concerns are addressed promptly Investigate and assist in resolving resident and family concerns in a professional and timely manner Promote a culture of dignity, respect, and individualized care for all residentsRequirements:
Current Maryland RN or LPN license (or compact/multi-state license) Current BLS/CPR certification Strong knowledge of MDS processes, nursing care practices, and regulatory requirementsPreferred Qualifications:
MDS Certification (AANAC)- preferred, not required Experience with PointClickCare (PCC) Prior experience in skilled nursing or long-term care Leadership or management experience Limited opportunities available
