MUST HAVE EXPERIENCE IN A SKILLED NURSING FACILITY

• Perdiem days and Evenings Shifts Position Summary Oversees daily nursing operations to ensure high-quality resident care in a skilled nursing facility. Supervises and supports nursing staff, including RNs, LPNs/LVNs, and CNAs. Ensures compliance with federal, state, and local regulations governing long-term care facilities. Promotes a safe, compassionate, and resident-centered care environment. Key Responsibilities Clinical Leadership & Supervision Supervise nursing staff during assigned shifts; provide guidance, coaching, and performance feedback. Make rounds to monitor resident condition and quality of care delivery. Assess resident needs and ensure appropriate nursing interventions are implemented. Respond to changes in resident condition and initiate appropriate action, including physician notification. Serve as clinical resource and mentor for nursing staff. Regulatory Compliance & Documentation Ensure compliance with CMS, state health department, and facility policies. Monitor accurate and timely completion of clinical documentation. Participate in surveys and inspections; assist with corrective action plans as needed. Maintain adherence to infection control protocols and safety standards. Care Coordination Collaborate with physicians, nurse practitioners, therapists, social services, and other interdisciplinary team members. Participate in care plan meetings and ensure nursing input is incorporated. Communicate effectively with residents and families regarding care and concerns. Oversee admissions, transfers, and discharges. Staffing & Operations Assist with scheduling and staffing assignments to ensure adequate coverage. Monitor workflow and adjust assignments based on resident acuity. Participate in hiring, onboarding, and training of nursing personnel. Address employee concerns and escalate issues to

as appropriate. Quality Assurance & Risk Management Monitor incidents and accidents; ensure proper documentation and follow-up. Participate in Quality Assurance/Performance Improvement (QAPI) initiatives. Ensure medication administration practices meet safety standards. Promote fall prevention, wound care management, and pressure injury prevention. Qualifications Active, unencumbered Registered Nurse (RN) license in the state of employment. Minimum of 2-3 years of nursing experience in long-term care or skilled nursing preferred. Prior supervisory or charge nurse experience strongly preferred. Current CPR/BLS certification. Knowledge of MDS process and long-term care regulations preferred. Skills & Competencies Strong leadership and team management skills. Excellent clinical assessment and critical thinking abilities. Effective communication and conflict resolution skills. Ability to multitask and manage priorities in a fast-paced environment. Proficient in electronic medical record (EMR) systems.
