Hartville Family Physicians is looking for a RN Chronic Care Manager with a passion for helping others, to coordinate patient services both within our practice as well as within the complex healthcare delivery system. Responsibilities:
Develop patient centered care plan by involving patients in decisions about their own health Conduct patient education Act as patient advocate Match patient needs with appropriate services based on consultation with practitioner Arrange post-care follow-up such as home care, therapy, palliative care, hospice for patients enrolled in care management Work collaboratively with referral department to coordinate referrals to specialists if needed Organizes patient outreach to facilitate follow-up care or services Manage daily schedule to ensure proper patient follow-up Concurrent documentation during management in patient charts
Requirements:
Registered Nurse (RN)-2 years minimum; hospital, home health, or skilled nursing facility experience Comprehensive knowledge of area resources; familiarity of medical practices for patient self-management; understanding of Medicare, Medicaid, and VA systems Ability to teach others, including patients, peers, and staff; critical thinking for individualized patient care Self-motivated; strong verbal and written communication skills; flexible; teamwork within individual offices and care management team
