We strive everyday to provide positive health, safety, and life outcomes for individuals with intellectual and developmental disabilities. 5 Star operates 13 residential group homes, a group day support, and a 24/7 private duty nursing service in the Tidewater and Richmond, VA areas. The Nurse Supervisor will lead a talented team of 20 to 30 Certified Clinical Medical Assistants, Certified Nursing Assistants, Registered Nurses, and Licensed Practical Nurses to ensure that the individuals who trust us with their care lead happy and fulfilling lives. The Nurse Supervisor will oversee the overnight operations of the Tidewater-area group homes where individuals with intellectual and/or developmental disabilities Duties and responsibilities: Care for individuals with ID-DD in a residential group home Manage and support Direct Support Professionals, CNA's, MA's, PCA's, LPN's, and RN's Conduct skills trainings to continue education for all floor staff Assist in training new hires Professionally communicating with resident families, case workers and support coordinators regarding any changes in health, health service plan changes, or any health concerns Professionally communicating with all medical professionals regarding anything related to the resident's health Reviewing and auditing Medication Administration Records on R-task Reviewing and auditing all Skin Assessment Forms, BM Logs, I&Os Forms, Controlled Medication Forms, Seizure Logs, Etc. Reviewing and auditing daily documentation, notes, and charting performed by floor staff Creating floor staff schedules Reviewing all schedules to ensure all shifts are filled Going to the hospital when a resident is sent out for a change in health status Coordinating and attending all medical appointments Attending all community outings Participating in new individual intake meetings and during admission (Inputting individual, all medications, service plan, medical orders, and medical contacts into R-task) Completing, reviewing, signing nursing plans of care Participating and assisting with hiring Knowledge skills and abilities: Effectively intercede with family and residents and resolve concerns related to the delivery of health care services. Develop, maintain, and evaluate resident/client and staff data collection systems and processes to support the practice of nursing and the delivery of resident care. Collaborate with residential staff at all levels, interdisciplinary teams, executive officers, and other stakeholders in the development, implementation, and evaluation of programs and services. Develop and implement recruitment and retention programs for Nursing staff. Delegate responsibilities appropriate to licensure, education, and experience of staff. Utilize evidence-based practice to develop and implement standards of practice that guide practice improvement initiatives. Recognize the diverse skill levels of individual staff members and the unique needs of the resident population when developing staffing plans; ensures level of staffing for the level of acuity is appropriate 24 hours/day. Evaluate staff competency by direct observation, peer review, orientation, and employee input, documenting this in the appropriate system. Develop and implement effective educational programs designed to assist the staff in the progression from novice to expert in the care of the resident and family. Training requirements : Must be trained in CPR, First Aid, Medication Administration Management, and Aggressive Behavior or Behavioral Supports.
