Evaluate patient's physical, cognitive, and psychosocial abilities. Develop and implement treatment plans to improve patient's quality of life. Provide instruction to patients and families on how to use assistive devices and adaptive techniques. Monitor patient progress and modify treatment plans as needed. Collaborate with other healthcare professionals to coordinate care for patients. Educate patients on self-care strategies and techniques. Provide support to family members of patients.
