Job Description

Deliver meaningful impact as a Home Health Occupational Therapist, providing personalized occupational therapy services to patients in their residences. Rely on your expertise to enhance patient independence and safety through holistic care and education tailored to home environments.
Schedule:
Monday Friday, 8:00 AM 5:00
PM Key Qualifications:
Active Occupational Therapist (OT) license in good standing (state of practice) At least one year of recent experience in Home Health OT (field background preferred) Proficiency with Home Care Home Base (HCHB) documentation system welcomed OASIS assessment experience required Demonstrated strength in clinical assessment, functional training, and care planning Valid drivers license, reliable transportation, and auto insurance Organized, self-sufficient, and comfortable managing an independent field-based caseload Excellent verbal and written communication abilities
Primary Responsibilities:
Conduct comprehensive occupational therapy evaluations, including OASIS assessments for various stages of care Design and execute individualized care plans focusing on ADLs, IADLs, safety, upper extremity function, and cognition Deliver skilled interventions such as self-care training, therapeutic activities, adaptive equipment training, and home modifications Assess and recommend appropriate adaptive or durable medical equipment Monitor progress, make timely updates to plans, and collaborate with interdisciplinary care teams Educate patients and caregivers on safety, functional strategies, and fall prevention Submit accurate, timely documentation in the HCHB system Attend case conferences and interdisciplinary meetings for coordinated care Adhere to agency protocols and regulatory standards
