Zenith Care Home Health, LLC Tamarac, FL | Broward County Zenith Care Home Health, LLC is seeking a compassionate, dependable, and professional Occupational Therapist (OT) to join our growing home health team. We are looking for OTs who are committed to helping patients safely participate in daily activities and improve their independence in their homes.

RESPONSIBILITIES
Evaluate patients' functional abilities and occupational performance. Develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services according to the plan of care. Assist patients with activities of daily living and functional tasks. Address mobility, coordination, strength, cognition, and other functional needs within the OT scope of practice. Recommend and educate patients regarding appropriate adaptive equipment and techniques. Evaluate the patient's home environment and identify potential safety concerns. Provide education to patients and caregivers regarding safety, independence, and functional activities. Monitor patient progress and modify treatment as clinically appropriate. Communicate patient progress and changes in condition with the physician and interdisciplinary team. Complete accurate and timely clinical documentation. Participate in care coordination and interdisciplinary communication. Follow Medicare, Florida, agency, infection prevention, and patient safety requirements. Maintain patient confidentiality and comply with HIPAA requirements.
QUALIFICATIONS
Current and unrestricted Florida Occupational Therapist (OT) license . Current BLS/CPR certification , if required by agency policy and applicable position requirements. Strong clinical assessment and communication skills. Ability to work independently in a home health setting. Reliable transportation and ability to travel to patient homes throughout Broward County. Ability to complete required documentation accurately and on time. Home health experience is preferred but not required.
