RESPONSIBILITIES
Evaluate patients' communication, speech, language, cognitive-communication, and swallowing needs as appropriate. Develop individualized treatment plans based on patient needs. Provide skilled speech-language pathology services according to the established plan of care. Provide appropriate therapeutic interventions within the SLP's scope of practice. Educate patients and caregivers regarding communication strategies, exercises, safety, and other appropriate interventions. Monitor patient progress and modify treatment as clinically appropriate. Communicate patient progress and significant changes with physicians and the interdisciplinary care team. Complete accurate and timely clinical documentation. Participate in care coordination and interdisciplinary communication. Follow Medicare, Florida, agency, infection prevention, and patient safety requirements. Maintain patient confidentiality and comply with HIPAA requirements. Provide culturally respectful and patient-centered care.
QUALIFICATIONS
Current and unrestricted Florida Speech-Language Pathologist license , as applicable. Current BLS/CPR certification , if required by agency policy and applicable position requirements. Appropriate professional credentials required for the position. Strong clinical assessment and communication skills. Ability to work independently in a home health setting. Reliable transportation and ability to travel to patient homes throughout Broward County. Ability to complete required documentation accurately and on time. Home health experience is preferred but not required.
