Job Description

Deliver high-quality occupational therapy services directly to patients in their homes, helping them maximize functional independence and overall safety. This field-based role focuses on evaluating and treating adults with diverse needs, tailoring care plans to support daily living and home safety. Youll play an essential part in a collaborative interdisciplinary care team, closely coordinating with fellow clinicians to ensure optimal outcomes.
Schedule & Setting:
Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Caseload of approximately 2532 points per week (typically 57 patient visits per day)
Field-based:
provide services in patient homes within an assigned territory Electronic documentation through Home Care Home Base (HCHB)
Qualifications:
Current, valid Occupational Therapist (OT) license for the state of practice At least 1 year of recent experience in home health occupational therapy, including field-based work Demonstrated proficiency with OASIS assessments (Start of Care, Resumption, Recertification, Discharge) Experience with Home Care Home Base (HCHB) preferred Reliable transportation, valid drivers license, and auto insurance Strong clinical assessment, organizational, and communication abilities Ability to work independently and adapt to travel-based schedules
Key Responsibilities:
Perform comprehensive OT assessments, including OASIS, and create individualized care plans Deliver skilled interventions focusing on
ADL/IADL
retraining, upper extremity function, and home safety enhancements Recommend adaptive equipment and durable medical equipment as appropriate Provide education to patients and caregivers on functional independence and safety strategies Ensure thorough, timely documentation and ongoing communication with the interdisciplinary team Participate in team meetings and maintain compliance with regulatory and agency standards Benefits include weekly pay, access to medical, dental, and vision insurance, 401(k) retirement plan options, 24/7 support, mileage reimbursement, and potential for contract extension.
