Massapequa Center Rehabilitation & Nursing is committed to improving our residents' quality of life and independence through exceptional rehabilitative services and compassionate, patient-centered care. We are seeking a diligent and skilled Occupational Therapist (OT) to join our interdisciplinary team at our skilled nursing facility in Amityville, NY . This is a full-time position with an 8:00 AM-4:00 PM schedule , with some flexibility. No weekends are required.

Job Responsibilities Include:

Provide occupational therapy services to residents Develop and improve the skills needed for residents to perform daily activities and maintain independence Accurately complete and document all treatment procedures and functional mobility tasks in a timely manner Establish individualized plans of care based on each resident's needs, abilities, and goals Apply appropriate interventions to maximize residents' performance in activities of daily living and instrumental activities of daily living Monitor resident progress and make adjustments to treatment plans as needed Prepare documentation, progress notes, reports, and discharge summaries as required

Qualifications Include:

Valid New York State Occupational Therapist license and credentials SNF/Long-Term Care experience preferred Minimum 1 year of occupational therapy experience
