Responsibilities Conduct comprehensive occupational therapy assessments to evaluate residents' functional abilities and needs. Develop and implement individualized care plans tailored to each resident's goals, medical conditions, and therapy requirements. Assist residents with activities of daily living (ADLs), such as dressing, grooming, feeding, and mobility to foster independence. Collaborate with interdisciplinary teams—including speech therapists, physical therapists, physicians, and educators—to coordinate holistic care strategies. Support discharge planning by preparing patients for transition back home or to other care settings through skill development and caregiver training. Manage medical documentation accurately within electronic health records (EHR) systems while ensuring compliance with clinical standards. Provide specialized interventions for individuals with autism spectrum disorders and other neurodevelopmental or developmental disabilities. Participate in case management activities to monitor progress and adjust treatment plans as needed for optimal outcomes. Implement occupational therapy programs across diverse environments such as inpatient units, outpatient clinics, schools, or home health settings. Support early intervention initiatives by working closely with children and families to promote developmental milestones. Maintain up-to-date knowledge of physiology and anatomy relevant to occupational therapy practices and treatment techniques. Engage in clinical interprofessional collaboration to deliver comprehensive patient-centered care that emphasizes evidence-based practices. Experience Proven experience working with individuals across a broad age range—including geriatrics, and young adults—with disabilities or neurodevelopmental disorders. Prior involvement in acute care settings such as hospitals or Short Term Rehab facilities. Board Certified in state of South Carolina. Experience working within skilled nursing facilities or post-acute care environments is highly desirable. Background in outpatient clinics or home health services supporting patient recovery and functional independence. Knowledge of medical terminology, physiology, anatomy, and clinical documentation standards necessary for accurate record keeping. Demonstrated ability to develop treatment plans based on evidence-based practice principles while utilizing electronic health records (EHR) management systems effectively.
