Duties Conduct comprehensive occupational therapy assessments to evaluate clients' functional abilities and needs Develop and implement individualized treatment plans focused on improving daily living skills, motor functions, and cognitive performance Assist clients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and personal hygiene Support discharge planning by coordinating care transitions and providing education to clients and caregivers Work closely with interdisciplinary teams—including speech therapists, physical therapists, and social workers, —to deliver integrated care Manage electronic health records (EHR) accurately to document assessments, treatment progress, and clinical notes Provide specialized interventions for geriatrics in post-acute care settings Participate in case management activities to monitor client progress and adjust treatment plans accordingly Educate clients and their families on therapy techniques and strategies to support ongoing development at home Engage in evidence-based practice to ensure the delivery of the highest quality occupational therapy services Requirements Valid licensure or certification as an Occupational Therapist in accordance with state or national regulations Proven experience working with individuals across various settings such as inpatient hospitals, skilled nursing facilities, and outpatient clinics. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards Experience conducting occupational therapy assessments for adults with neurodevelopmental disorders or physical rehabilitation needs Ability to develop effective treatment plans aligned with client goals and medical care objectives Skilled in implementing occupational therapy programs tailored to adults requiring post-acute rehabilitation Excellent communication skills for client interaction and clinical interprofessional collaboration Familiarity with electronic health records (EHR) management systems for documentation purposes Knowledge of autism spectrum disorders support strategies and working with individuals with neurodevelopmental challenges Acute care experience is highly desirable Outpatient or clinic experience supporting patients through evidence-based occupational therapy interventions is preferred Embark on a rewarding career where your expertise transforms lives!
