IN - HOUSE
Occupational Therapist for an Assisted Living Facility !! A Lot of flexibility. We are seeking a passionate and dedicated Occupational Therapist to join our dynamic healthcare team that we are building at this time. In this vital role, you will assess, plan, and implement personalized therapy programs to help patients within The Victorian to regain independence and improve their quality of life. This position offers an exciting opportunity to collaborate with multidisciplinary teams, develop innovative care plans, and make a meaningful difference in the lives of individuals with diverse needs within our skilled nursing facility. Responsibilities Conduct comprehensive evaluations of patients' physical, cognitive, and emotional abilities to determine their therapy needs. Option for Telehealth Evaluations will be available as well. Develop and implement tailored treatment plans focusing on improving ADLs (Activities of Daily Living), mobility, coordination, and functional independence. Assist patients with coordinating care transitions and providing education for ongoing therapy at home or in community settings. Manage case documentation accurately, including progress notes, care plans, and treatment reports while ensuring compliance with healthcare regulations. Collaborate with physicians, speech therapists, nurses, social workers, and other healthcare professionals to deliver holistic patient care. Qualifications Bachelor's or Master's degree in Occupational Therapy from an accredited program; licensure or certification as required by state or country regulations. Strong knowledge of physiology, anatomy, medical terminology, and patient care principles essential for effective therapy planning. Experience developing care plans and managing case loads across multiple settings including outpatient clinics, skilled nursing facilities, schools, and hospitals. Excellent communication skills to educate patients and families about therapy goals and discharge instructions. Experience in Caseload Development is a plus! Ability to adapt quickly to different environments while maintaining a positive attitude and energetic approach.
