The ideal candidate will have experience in home health and be able to provide quality care to patients in their homes.
Responsibilities:
Assess patients' needs and develop individualized treatment plans Provide therapy services to patients in their homes Monitor patients' progress and adjust treatment plans as needed Collaborate with other healthcare professionals to ensure comprehensive care Document patient progress and maintain accurate records Provide education to patients and their families on therapy techniques and home exercises
Requirements:
Valid Occupational Therapist (OT) license Experience in home health Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong problem-solving and critical thinking skills Ability to travel to patients' homes
