You'll primarily work in a home health setting , with occasional visits in skilled nursing facilities. What You'll Do Work independently with strong self‑direction and minimal supervision Review and respond to patient referrals Collaborate with the clinical team to educate patients and caregivers Develop and implement individualized treatment plans with functional, achievable goals Provide instruction and supervision to COTAs Assist with identifying and recommending appropriate DME (wheelchairs, adaptive equipment, etc.) Perform additional responsibilities as needed to support patient care Why Join PRN Home Health & Therapy?
