Key Responsibilities:

Conduct initial and ongoing assessments of mobility, strength, and functional ability. Develop and implement individualized plans of care with measurable goals and outcomes. Deliver occupational therapy services per physician orders, documenting progress and modifying treatment plans as needed. Coordinate discharge planning, including patient and caregiver education on home exercises. Collaborate with healthcare teams, including physicians and nurses, to ensure comprehensive care. Maintain accurate documentation to support billing and compliance, including OASIS assessments where permitted. Supervise other personnel such as OTA and HHA, if applicable.

Qualifications and Skills:

Degree in Occupational Therapy from an accredited program. Current, unrestricted state license as an Occupational Therapist. Valid CPR certification. Preferred experience in home health therapy and OASIS documentation.
