Community Brain Injury Specialist -
Cognitive Rehabilitation, Part-Time Location:
Hawley, PA & Surrounding Areas (In-Person Field Sessions)

Our mission is to create new opportunities for individuals affected by brain injury—one individual, one family, one Success at a time. We are looking for a goal-driven Community Brain Injury Specialist to support clients in the Hawley, PA region. Role Overview This is a specialized clinical skill-building role working 1-on-1 with adults recovering from traumatic brain injury (TBI) or acquired brain injury (ABI). You will empower clients to rebuild executive functioning, daily routines, and community connections.
Please Note:
This is a clinical rehabilitation position focused on cognitive and goal-oriented skill building. This is NOT a personal care, companion, or home health aide position.
Work Structure & Hybrid Flexibility In-Person Field Work:
Sessions are held directly in clients' homes and local Hawley/Pike/Wayne County community settings.
Remote Work:
Administrative duties, case documentation, team consultations, and ongoing training are completed from your home office.
Schedule:
Part-time, flexible scheduling built around client session needs. Key Responsibilities Deliver individualized cognitive rehabilitation sessions in real-world home and community settings. Guide clients through compensatory tools for memory, attention, planning, and task execution. Help clients establish daily self-care routines, appointment tracking, and household management strategies. Foster emotional regulation, frustration tolerance, and self-advocacy. Encourage safe participation in local community activities and social roles. Complete clinical progress notes promptly and participate in virtual care coordination meetings.
Qualifications Education:
Bachelor's degree in Psychology, Social Work, Human Services, Education, Rehabilitation Science, or a related field.
Experience:
Experience in brain injury rehabilitation, behavioral healthcare, disability services, or community mental health.
Requirements:
Valid driver's license, reliable personal transportation, and active insurance for client-site travel.
Skills:
