Linwood Meadows Care Center- Visalia, CA Salary range: $40
- 45/hour BOE Linwood Meadows Care Center, a top-rated skilled nursing facility in Visalia, CA, is seeking a Full-Time Certified Occupational Therapy Assistant (COTA) to join our compassionate and collaborative rehab team.
Position Summary:
The Certified Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to provide rehabilitative services to residents, helping them regain independence in activities of daily living and functional mobility.Key Responsibilities:
- Provide direct patient care as outlined in the treatment plan developed by the Occupational Therapist
- Assist in the implementation of treatment goals and plans
- Monitor patient progress and report changes to the Occupational Therapist
- Accurately document treatments and patient responses using facility systems (e.g., Casamba, PointClickCare)
- Educate residents, families, and staff on therapy goals and home exercise programs
- Participate in team meetings, care planning, and discharge planning as needed
- Ensure compliance with federal, state, and facility regulations and policies
Qualifications:
- Associate's degree from an accredited Occupational Therapy Assistant program
- Current COTA license in good standing with the State of California
- Experience in a skilled nursing or post-acute care setting preferred
- Strong communication and interpersonal skills
- Proficient in documentation software and electronic health records
