Job Description
Hospice of Lake Cumberland is looking to hire a LPN for our Crisis Care team. This position will be a dayshift part-time position with hours being from 8 am-8 pm. The schedule will be every Friday and every other weekend. Total hours will be 24 hours plus drive time from designated Hospice office. This position may travel to any of our seven counties which include Casey, Clinton, Cumberland, McCreary, Pulaski, Russell and Wayne. The applicant can reside in any of those counties as Crisis Care case changes frequently. Job duties will include one on one patient care in the home setting; teaching patients & families about end of life care and the needs of the patient; and collaboration with case manager regarding symptom management of the patient.
