The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care. We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Michigan in Bloomfield Hills, MI . This is a full-time, on-site position working from Monday-Friday from 8:00AM - 4:30 PM schedule with some holidays required. Key Responsibilities Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care. Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care. Assist patients with understanding their medical care, insurance coverage, and available financial resources. Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination. Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system. Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care. Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery. Educate patients on treatment protocols, self-care strategies, and medication usage. Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans. Advocate for patients' needs and help resolve any issues or barriers to care. Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
