Job Description

Care Coordinator / Group Facilitator Overview We are seeking a dedicated and compassionate Care Coordinator / Group Facilitator to support patients receiving residential treatment for substance use and mental health conditions. This hybrid position focuses primarily on facilitating engaging, structured groups that help patients develop practical life skills, strengthen coping strategies, build meaningful connections, and actively participate in their recovery. The role facilitates the 1:00 p.m. Life Skills Group, the 2:00-4:00 p.m. Therapeutic Activities Group, and scheduled recovery-focused step work groups. Step work draws on principles of the AA 12 Steps, using inclusive language and concepts relevant to both mental health and substance use recovery, such as self-reflection, accountability, acceptance, healthy relationships, and personal growth. Following group facilitation and associated documentation, responsibilities transition after 4:00 p.m. to case management and care coordination. This includes meeting with patients to identify needs and barriers during treatment, assisting with employment and benefits matters, communicating with families and referring providers, and collaborating with the treatment team to support continuity of care. Roles and Responsibilities Group Facilitation and Patient Engagement Prepare and facilitate the daily 1:00 p.m. Life Skills Group and 2:00-4:00 p.m. Therapeutic Activities Group using approved curriculum and purposeful activities appropriate to patients' needs. Facilitate scheduled step work groups that translate recovery principles into practical tools applicable to both mental health and substance use treatment. Teach and reinforce skills such as communication, emotional regulation, problem-solving, healthy boundaries, daily routines, and management of everyday responsibilities. Create a welcoming, respectful group environment that encourages participation, connection, and opportunities to practice new skills. Adapt group content and activities to varying abilities, readiness, and levels of engagement while maintaining structure and purpose. Actively engage patients, address barriers to participation, and redirect disruptive behavior with respect and consistency. Complete accurate, timely group documentation, including attendance, content provided, individual participation and response, and relevant observations. Communicate concerns about patient behavior, engagement, or changes in presentation to the appropriate treatment team members. Case Management and Care Coordination Complete case management responsibilities after 4:00 p.m., including initial patient meetings, follow-up tasks, documentation, and family or referent communication. Meet with newly admitted patients to identify practical needs and barriers during treatment, including employment, Family and Medical Leave Act (FMLA) paperwork, short-term disability (STD), financial concerns, legal obligations, custodial matters, and insurance or benefits coordination. Assist patients with obtaining and organizing necessary paperwork, tracking deadlines, and coordinating with authorized parties. Facilitate completion of forms requiring clinical or medical certification by the appropriate provider. Maintain clear follow-up plans for identified needs and communicate outstanding concerns to the treatment team. Provide family, support-person, and referent touchpoints to gather relevant information, communicate appropriate updates, and support patient engagement within the scope of the role. Ensure releases of information are accurate, current, and appropriate before communicating protected patient information. Collaborate with the treatment team, care coordination staff, external providers, and third-party payors to support treatment needs, aftercare planning, and transitions through the continuum of care. Connect patients with community resources that support stability and continued recovery. Document patient contacts, identified needs, interventions, external communications, and follow-up plans thoroughly and promptly in the electronic medical record. Team Collaboration and Patient Support Collaborate with clinical, nursing, support services, and care coordination staff to reinforce individualized treatment goals and promote consistent patient support. Participate in interventions when patients express a desire to leave treatment prematurely, helping identify concerns, address practical barriers, and involve the appropriate team members. Support de-escalation and crisis response within the scope of training and promptly escalate safety or clinical concerns. Orient patients to relevant programming and facility expectations and consistently reinforce respectful behavior and community standards. Maintain professional boundaries, patient confidentiality, and a compassionate, recovery-oriented approach. Prioritize scheduled group preparation, facilitation, patient engagement, and associated documentation while coordinating case management follow-up with the team.
