Job DescriptionWe're seeking a full-time Pharmacist Floater who would be based in Fort Lupton and be able to travel to our other SALUD Pharmacies located in Brighton, Commerce City, Longmont, and Fort Collins, based on where assistance is needed.

The Pharmacist approaches his/her tasks in a team-based care fashion that supports patients and their families in a self-management, self-efficacy, and behavior change. QualificationsEducation:

Bachelor's degree in Pharmacy, Pharm D preferred

Licensure:

Possess a license to practice pharmacy in good standing in the

State of ColoradoExperience:

Experience working at a Community Health Center pharmacy is preferred

Knowledge, Skills, and Abilities:
