Job Description

Category Pharmacy - Clinical Pharmacist Job Type Direct Hire This full-time, day-shift clinical pharmacist position specializes in oncology care within a hospital setting focused on comprehensive cancer patient treatment. The role involves working collaboratively with oncologists, nurse practitioners, and physicians to optimize chemotherapy and supportive care regimens for both inpatient and outpatient populations. Key responsibilities include establishing collaborative practice programs in oncology, participating in multidisciplinary rounds, reviewing and updating oncology care plans in the Electronic Health Record, and providing patient-specific medication education utilizing the teach-back method. The pharmacist will also serve as a subject matter expert for oncology fellows during their training, lead staff development initiatives, and contribute to continuous quality improvement activities within the oncology service line. Precepting pharmacy residents and students is an integral part of this role.
Qualifications:
Doctorate in Pharmacy (PharmD) required Post-graduate Year 1 (PGY-1) residency or equivalent clinical experience with at least three years in oncology practice; or Post-graduate Year 2 (PGY-2) residency in Oncology and/or equivalent experience Licensed Pharmacist in good standing Board Certified Oncology Pharmacist (BCOP) preferred Minimum three years of hospital and clinical oncology experience is required; five years preferred Experience with collaborative oncology practice and familiarity with national evidence-based guidelines
