Job Description

Job Overview We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will have a strong background in pharmacy practice, with experience in various settings including acute care and long-term care (LTC) pharmacy. This role involves providing exceptional patient service, conducting patient assessments, and ensuring the safe and effective use of medications. The Staff Pharmacist will collaborate with healthcare professionals to deliver optimal patient care and manage medication therapy. Duties Review and interpret medication orders for accuracy and appropriateness. Prepare and dispense medications, including compounding when necessary. Conduct patient assessments to evaluate medication therapy outcomes. Provide clinical pharmacy services, including pain management and immunizations. Manage inventory effectively to ensure adequate stock levels of medications. Utilize pharmacy software, EMR systems, and EHR systems for documentation and patient management. Educate patients on medication administration, potential side effects, and proper use of medications. Ensure compliance with HIPAA regulations and maintain patient confidentiality. Collaborate with healthcare teams to develop individualized medication management plans. Participate in quality improvement initiatives within the pharmacy department. Requirements Active pharmacist license in the state of practice. Experience in LTC pharmacy or critical care preferred; hospital experience is a plus. Completion of a pharmacy residency or equivalent clinical pharmacy experience is highly desirable. Strong knowledge of medical terminology, physiology, anatomy, and aseptic technique. Proficiency in using pharmacy software and managing inventory systems. Experience with IV insertion, IV infusion, and medication administration is beneficial. Excellent communication skills with a focus on patient service and care. Ability to work collaboratively within a multidisciplinary team to improve patient outcomes. Familiarity with pain management protocols and acute care settings is advantageous.
