Job Summary and Qualifications We are currently seeking a Clinical Pharmacist to join our dynamic team. Our clinical pharmacist would be an integral part of advancing clinical pharmacy practice and improving patient outcomes. What you will do in this role: Support the development, implementation, and maintenance of clinical pharmacy programs to ensure clinical services meet the requirements and needs of the patient population and community. Comply with facility policy and procedures and regulatory requirements Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system. Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, antimicrobial management, and clinical programs defined by HCA. Suggest appropriate, cost-effective therapeutic medication alternatives to prescribers, sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting appropriate drug therapy selection. Document all clinical activities and interventions accurately and completely. Oversee the quality of technicians work to ensure appropriateness of drugs ordered, dosage, route and interactions. Review and adjust medications of dosage/schedule as appropriate for patients to improve their condition. Monitor narcotic transactions and support the controlled substance program by dispensing and reconciling the quantity/type of narcotics leaving the pharmacy. Accurately compound and dispense pharmaceuticals as needed. Provide accurate and timely medication information to healthcare staff and patients. Support and participate in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to reporting medication errors and adverse drug reactions, National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives. Maintain professional competency. Participate in the training of new pharmacists, pharmacy students, and technicians What qualifications you will need: Bachelor of Science or Pharm D degree in Pharmacy from College of Pharmacy Experience as a pharmacist in an acute-care hospital setting or general pharmacy practice residency preferred. Current FL Pharmacist license or license eligible. Eligible to sit for North American Pharmacist Licensure Exam (NAPLEX). Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.
