Required Qualifications:

The minimum qualifications for the position of Pharmacist are: Must be at least 18 years of age Must be a graduate of a school of pharmacy Must have and maintain an unrestricted, state pharmacist license Must not have active suspensions, sanctions, or other restrictions related to participation in a private, federal, or state health insurance program upon hire and thereafter and

Key Responsibilities :

Lead Collaborative Practice Agreements (CPA) and manage medication therapy for patients in care homes or on complex regimens Perform Medication Therapy Management (MTM) reviews via OutcomesMTM, Mirixa, and/or internal consults Oversee a clinical technician/medication coordinator responsible for daily communication with care facilities Develop and refine clinical SOPs , intervention tracking, and quality assurance systems Act as a liaison between homes, prescribers, and caregivers for med changes, labs, or clinical escalations Participate in audits and ensure HIPAA and compliance best practices Preferred Experience & Skills Licensed Pharmacist in the State of Georgia Experience in community care, ambulatory care, psychiatric pharmacy, or pain management Prior exposure to LTC, hospice, or complex home-based patients Knowledge of polypharmacy risk, antipsychotics, mood stabilizers, opioids MTM-certified or willing to complete APhA certificate Professional and compassionate communicator with a team-first attitude
