Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A home infusion clinical pharmacist plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications to support patient care in the home setting. Acting as the central hub within a multidisciplinary team, this position ensures sterile compounding practices, optimizes medication dosing, and delivers essential education to patients and caregivers.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions under sterile conditions, adhering to state and federal regulations. Assess laboratory results, calculate pharmacokinetic dosing (e.g., for vancomycin), and monitor Total Parenteral Nutrition (TPN) to adjust patient treatments as needed. Provide thorough patient counseling and education on specialty medications, infusion pump usage, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and various support teams (intake, referral, reimbursement, nursing, clinical, and pharmacy operations) to ensure coordinated therapy management and prevent hospital readmissions. Maintain regulatory compliance with State Board of Pharmacy guidelines, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards. Manage pharmacy inventory, including refrigerated products, and oversee the packing and shipment of medications and supplies for safe delivery to patients' homes.
Qualifications:
Current and valid New York pharmacist license in good standing. Minimum of 2 years' experience in infusion specialty pharmacy. Experience with IVIg therapy preferred. Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations. Familiarity with CareTend system is a plus.
