Job Description
We are seeking qualified Pharmacist to service our patients though out the Charleston area. Pharmacists play a vital role in ensuring the safe and effective use of medications to improve patient outcomes. You will collaborate with interdisciplinary care teams to optimize medication therapy and promote medication adherence. Your responsibilities include medication management, patient consultations, care coordination, and care management, all aimed at enhancing patient care and reducing rehospitalizations. Our pharmacist conducts virtual patient visits from the comfort of their own homes . This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
