Job Overview We are seeking a highly skilled and dedicated Pharmacist to join our healthcare team. Responsibilities Dispense medications accurately and efficiently using pharmacy software and electronic health record (EHR) systems Conduct patient assessments to determine appropriate medication therapy and provide counseling on medication use, side effects, and interactions Prepare and compound medications, including sterile preparations such as IV infusions, with strict adherence to aseptic techniques and safety protocols Manage inventory levels of pharmaceuticals, ensuring proper storage, stock rotation, and compliance with regulatory standards Administer immunizations and provide patient education on vaccination schedules and health maintenance Collaborate with healthcare providers to develop individualized treatment plans for diverse patient populations including pediatrics, LTC residents, and critical care patients Maintain compliance with HIPAA regulations to ensure patient confidentiality and data security Assist with medical management tasks such as pain management protocols, medication administration procedures, and clinical documentation Support pharmacy technician staff by overseeing workflow, verifying prescriptions, and ensuring accuracy in dispensing processes Requirements Valid pharmacist license with current registration in the state of practice Experience in LTC pharmacy, critical care, or hospital settings preferred; residency or clinical pharmacy experience is highly desirable Proficiency with pharmacy software systems, EMR/EHR platforms, and inventory management tools Knowledge of compounding medications, aseptic techniques, IV insertion, IV infusion preparation, and medication administration protocols Strong understanding of physiology, anatomy, medical terminology, and pharmacology principles Experience with immunizations administration and patient care services Ability to perform patient assessments effectively while demonstrating excellent communication skills Familiarity with HIPAA regulations to ensure compliance in all aspects of patient information handling Previous pharmacy technician experience is advantageous but not mandatory; relevant certifications are preferred This role is integral to delivering comprehensive pharmaceutical care that enhances patient health outcomes through expert medication management and collaborative healthcare practices.
