Job Description
Minimum Requirements:
- A Master's Degree in Counseling or Social Work
- Currently licensed in Texas as an LPC in Texas or as a
LMSW/LCSW
- Experience working with clients who have experienced trauma or are at-risk
- Knowledge & training in evidenced-based practices and Trauma Informed Care
- Culturally competent and committed to serving diverse populations with integrity and respect
- Experience working with 18
- 24 year olds: 2 years (Required)
Position Summary:
Responsible for providing trauma informed clinical services to both day and housed clients in the Transitional Living Program. Provide clinical therapy, crisis intervention, and psychoeducation on life skills to help at-risk youth move from crisis to self-sufficiency. Essential Duties and Responsibilities include the following but are not limited to:- Adhere to The American Counseling Association/NASW Code of Ethics
- Maintain personal and professional boundaries with clients at all times
- Maintain current licensure and provide the Human Resources Department with proof of current licensure
- Maintain current Professional Liability insurance
- Completes comprehensive assessment within program guidelines and formulates a diagnostic impression by conducting client sessions and identifying strengths, challenges, and needs, for the development of a treatment plan
- Prepares individualized treatment plan, in conjunction with client to include goals, interventions, necessary support, and include any referrals for additional services
- Engage in ongoing collaboration with other team members
- Conduct individual counseling sessions with caseload of 15 clients in accordance with treatment plan or as necessary for client crisis management
- Conduct mental health evaluations for prospective clients to assess if within scope of care for the Transitional Living Program.
- Engage in crisis intervention and continue to assess for risk of harm with all clients
- Remain informed and current on all aspects of trauma informed counseling techniques
- Attend weekly staffing meetings to enhance client continuity of care and weekly housing meetings to discuss program applicants
- Ensure that all client files are up to date on case notes, assessments, treatment plans, etc., as well as HIPPA compliant
- Prepares and conducts group counseling and or workshops as needed and decided on by the clinical team
- Be part of client teams helping with crisis interventions and court directives
- Attend regular treatment team staff meetings to enhance client continuity of care
- Enter all client services provided into the data base daily
- Provide CQI information and outcomes to supervisor on a monthly basis
- Attend monthly counseling huddle meetings and staff meetings
