Job Description
Job Overview We are seeking a highly motivated pharmacist to join our Internal Medicine/Trauma Clinical Pharmacist team. In this vital role, you will provide pharmaceutical care to patients within a fast-paced hospital environment, ensuring optimal medication therapy and patient safety. This position offers an exciting opportunity to apply your clinical knowledge, foster patient-centered outcomes, and contribute to innovative medication management practices. Duties Collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement individualized medication plans. Conduct comprehensive patient assessments, including physiology and anatomy knowledge, to determine appropriate pharmacotherapy strategies. Manage complex medication regimens Ensure pharmaceutical regulatory compliance by adhering to drug regulation standards and hospital policies. Utilize Pharmacy Information Systems (PIS) for medication ordering, verification, and documentation to streamline clinical workflows. Precept pharmacy residents and students, providing guidance on clinical practices. Monitor patient responses to therapies, adjusting medications as needed to optimize outcomes while minimizing adverse effects. Maintain aseptic techniques during preparation of medications and ensure adherence to infection control protocols. Stay current with advances in pharmacology Skills Desired Pharmacy residency experience (PGY1 and/or PGY2) and/or experience as a clinical pharmacist in an acute care setting Experience in managing patients medication therapies such as Vancomycin, warfarin, TPNs, aminoglycosides, renal dose adjustments, antibiotics, etc. Desires to precept pharmacy students or residents in clinical practice settings.
