Operate medical equipment safely and effectively during therapy sessions, ensuring proper use and maintenance. Document clinical assessments, treatment progress, and discharge plans accurately in electronic health records (EHR) following clinical documentation standards. Collaborate closely with healthcare professionals such as physicians, occupational therapists, and nurses to coordinate patient care plans and optimize outcomes. Communicate clearly with patients and caregivers about treatment goals, procedures, and home care instructions in both English and Spanish to ensure understanding and compliance. Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up therapies or referrals. Experience Proven experience in physical therapy or as a physical therapist assistant within home health environments. Demonstrated knowledge of physiology, anatomy, orthopedics, pediatrics, geriatrics, and acute care practices relevant to diverse patient populations. Skilled in clinical assessment documentation and medical terminology necessary for accurate record-keeping and reporting. Familiarity with medical documentation standards and electronic health records (EHR) management systems. Hands-on experience with manual therapy techniques such as trigger point therapy and orthopedic physiotherapy interventions. Ability to communicate effectively with patients of all ages in both Spanish and English while maintaining compassionate caregiver communication. Prior involvement in discharge planning processes for clients transitioning between care settings is highly desirable.
