  • 50/hour | Full-Time | Benefits Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center Cheshire.
This full-time opportunity offers the chance to work directly with residents while helping them improve mobility, rebuild strength, and achieve greater functional independence in a skilled nursing and post-acute care environment. What You'll Do Perform physical therapy evaluations and develop individualized plans of care Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility Establish functional goals and monitor resident progress throughout treatment Modify treatment approaches based on clinical findings and resident needs Educate residents and caregivers on exercises, mobility strategies, and safety techniques Complete accurate and timely clinical documentation Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team What We Offer $45
