Work Environment In this home care setting, your primary work location will be our patients' residences, allowing you to deliver personalized treatments in a familiar and comfortable environment. You will work closely with an interdisciplinary team, including medical professionals and support staff, to ensure a holistic approach to patient care. We maintain a drug-free workplace and prioritize various vaccinations to ensure the safety of our staff and patients. Key Responsibilities Conduct comprehensive patient assessments to analyze functional limitations and disability causes. Collaborate with healthcare providers to establish a continuum of care ensuring an integrated approach to medical interventions. Develop, implement, and document individualized treatment plans incorporating evidence-based practices. Educate patients and their families about treatment options, care instructions, and discharge plans. Regularly evaluate treatment efficacy, adjusting strategies based on patient progress and feedback. Utilize appropriate software and electronic documentation systems to maintain detailed and compliant patient records. Participate in community outreach initiatives aimed at promoting health and wellness. Engage in continuous learning through staff meetings, in-services, and professional development opportunities. Supervise and mentor support staff to ensure quality service delivery for all patients. Patient Population / Client Focus Our patient population is diverse and includes individuals recovering from surgery, managing chronic conditions, or rehabilitating from injuries. Qualifications Bachelor's Degree in Physical Therapy from an accredited program. Valid state license to practice Physical Therapy in Florida. Basic Life Support (BLS) certification. Preferred Experience While a minimum of two years of experience in a clinical setting is required, candidates with experience in home health care or outpatient therapy environments will be favored.
