Job Function:
Provides and directs Physical Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under physicians' orders.Responsibilities Include:
- Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.
- Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
- Evaluates residents to obtain data necessary for treatment planning and implementation.
- Conduct specialized evaluations as indicated.
- Adheres to established confidentiality standards.
- Implements and conducts treatment as outlined in treatment plan.
- Follows management direction.
- Performs other duties as assigned.
Education and Experience:
- Bachelor of Science in Physical Therapy from an accredited program.
- Successful completion of National Certification Examination for Registered Physical Therapist.
- Current state licensure or license eligible.
- 0-3 years of long-term care experience.
Background Screening Requirement:
This position requires background screening through the Agency for Health Care Administration (AHCA) Care Provider Background Screening Clearinghouse.Learn more:
