Oakland, Wayne, and Macomb Counties Pay Range:
$75

  • One-on-one patient care Position Overview We are seeking a motivated Physical Therapist to provide home-based care to patients with a variety of functional impairments.
This role involves evaluating patients, developing individualized treatment plans, and delivering high-quality care aimed at maximizing independence and mobility. You'll also collaborate with the broader care team and maintain accurate clinical documentation. Key Responsibilities Perform initial evaluations and ongoing assessments to determine patient needs Develop and implement personalized treatment plans Deliver skilled therapy services focused on strength, mobility, and functional outcomes Complete documentation in a timely and compliant manner Communicate effectively with patients, families, and interdisciplinary team members Supervise PTAs and therapy aides when applicable Participate in care coordination and case discussions as needed Qualifications Graduate of an accredited Physical Therapy program Active Michigan Physical Therapist license Home health experience preferred Skills & Attributes Strong clinical decision-making and problem-solving ability Self-motivated and comfortable working independently Excellent communication and interpersonal skills Familiarity with EMR systems for efficient documentation and communication within healthcare teams. Kinnser/Wellsky EMR experience preferred. Ability to safely assist patients with mobility and transfers (with appropriate equipment)
