The gait doesn't match what was described in the referral. The patient says they haven't fallen, but they're reaching for furniture every few steps. The spouse is listed as the primary caregiver, but you're beginning to wonder whether they're physically capable of providing the assistance the patient needs. The numbers matter. The documentation matters. The standardized assessments matter. But so does clinical judgment developed through years of actually treating patients. Home Health Care, Inc. is hiring an experienced Physical Therapist (PT/DPT) to focus on Start of Care admissions throughout the Twin Cities Metro and surrounding communities. Put Your Assessment Skills at the Center of Your Work This isn't a traditional PT role centered around carrying a routine treatment caseload. Your primary responsibility is figuring out where the patient is starting. You'll enter a new patient environment, perform a comprehensive assessment, establish the functional baseline, complete OASIS documentation, identify risks, determine appropriate therapy needs, and help build the plan of care. Then you'll do it again with an entirely different patient and clinical picture. The next may have experienced a stroke. Another could be dealing with repeated falls, progressive weakness, Parkinson's disease, cardiac or pulmonary limitations, or significant deconditioning following hospitalization. There's no established treatment rhythm when you arrive. You're establishing it. You'll determine what the patient can safely do today, what they're struggling with, what risks exist, what support is available, and what needs to happen next. That requires more than checking boxes. It requires a Physical Therapist who can observe, question, assess, connect information, and make sound clinical decisions. What You'll Be Doing Complete comprehensive Start of Care Physical Therapy evaluations Perform and document required OASIS assessments Assess gait, balance, strength, endurance, transfers, mobility, and fall risk Establish functional baselines and patient-centered goals Evaluate caregiver support and the patient's home environment Identify immediate safety and mobility concerns Develop individualized plans of care based on assessment findings, physician orders, and patient goals Determine appropriate therapy frequency and interventions Recommend assistive devices and durable medical equipment when appropriate Provide initial patient and caregiver education Coordinate with nursing, OT, SLP, social work, physicians, and other clinicians Complete accurate and timely electronic documentation This Role Rewards Experience Start of Care isn't simply a longer therapy evaluation. You're gathering information that helps shape an entire episode of home health care. That's why we're especially interested in PTs who already understand home health, OASIS, admissions, Medicare requirements, and comprehensive assessment. You should be comfortable walking into an unfamiliar situation without an established treatment routine and confidently determining what needs attention. What You Bring Current Minnesota Physical Therapist (PT) license Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong comprehensive assessment and clinical reasoning skills Excellent documentation and organizational skills Strong patient, caregiver, physician, and interdisciplinary communication Ability to independently manage a field-based schedule Especially Valuable Experience Previous home health experience OASIS experience Start of Care or admission experience Knowledge of Medicare home health requirements Experience with orthopedic, neurological, cardiac, pulmonary, geriatric, or medically complex patients Optional Weekends Give You Another Earning Lever Your primary schedule is during the week, with Start of Care visits paying $100 per admission plus mileage reimbursement . If you'd like to pick up additional admissions, optional weekend Start of Care visits may be available at $120 per visit . Weekend visits are optional, and there are no holiday requirements.
