Job Description
Overview Join our dynamic healthcare team as a Licensed Physical Therapist and make a meaningful difference in patients' lives! Your expertise will support patients across various settings, including outpatient clinics and, home health environments. . Duties Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and neurological function Develop and implement customized treatment plans targeting orthopedic, neurological, pediatric, and geriatric patients Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care plans and optimize patient outcomes Manage discharge planning processes by preparing detailed reports and recommendations for ongoing care or community services Document all patient interactions accurately in electronic medical records (EMR) systems following clinical documentation standards Provide patient education on injury prevention, treatment exercises, and home care strategies to support rehabilitation outside clinical settings Interest in neurological conditions would help grow this role within our practice Requirements Valid state licensure ND and MN as a Physical Therapist with practicing rights in relevant healthcare settings Demonstrated experience in outpatient clinics or home health services with a focus on patient rehabilitation Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards Proven ability to develop effective treatment plans for diverse populations including geriatrics, pediatrics, orthopedics, and neurology patients Experience working with electronic medical records (EMR) systems for accurate documentation Excellent client communication skills to educate patients and collaborate with multidisciplinary teams Ability to perform discharge planning efficiently while adhering to healthcare protocols Familiarity with outpatient rehabilitation center practices and inpatient hospital procedures is preferred Embark on a rewarding career where your expertise fuels recovery journeys!
